Position Summary

Home & Community Care Program for Younger People Assistant

Casual- until June 2022

Our Home and Community Care Program for Younger People provides critical support to young people living with disability and their families/carers.

The Home & Community Care Program for Younger People Assistant is a newly established position designed to ensure an efficient and effective service to our community and ensure the smooth running of internal processes, such as: 

  • Arranging suitable community care services for clients in accordance with service plans and as directed by the HACC PYP Assessment Officers
  • Preparing and distributing rosters to community care staff
  • Understanding the individual capabilities of our community care workers to allocate clients appropriately

The successful applicant will have high level communication skills with the ability to interact with a diverse range of community members, have excellent administration and time management skills and have experience in using the Sharikat system.
